Customer Application

MINIMUM DELIVERY

A minimum of 300 gallons is required for all deliveries. Any deliveries made under 300 gallons are subject to extra charges and a delivery fee.

PAYMENT TERMS

All purchases made during the month are due upon the receipt of the delivery.

FINANCE CHARGES

After 10 days if no payment is made, your balance will increase by $0.07 per gallon. If after 30 days no payment is made, your balance will increase again with a finance charge that will generate every 30 days.

PAST DUE ACCOUNTS

Any account not paid within 90 days of the delivery date will be placed on COD (cash on delivery). All sales and service from that time forward will be handled on a cash-only basis, unless other arrangements are made with management.

REMOVAL FROM ROUTE

Accounts who have an outstanding balance will not receive a delivery until all balances are paid in full. Failing to pay for your past deliveries will result in you being removed from your route list. If you are sent to collections or become past due you will need to pay for ALL future deliveries in advance.

COLLECTIONS POLICY

Should the account remain past due for 2 months after the statement date, a collections agency, lawyer, or small claims court will be used as sources of collection. Once this happens, we will no longer make deliveries unless you pay in advance. Note if you are a route customer you will be removed from the list. You will need to call in to place new orders and PRE-PAY.

ALL DELIVERIES

We ask that all previous deliveries be paid for before your next delivery.

PAYMENT METHODS

We accept cash, check, PHILIPS 66, VISA, MASTERCARD, DISCOVER, and AMERICAN EXPRESS as payment on your account.

AGREEMENT

I verify that all the above information is correct. I, the undersigned, give Jay's Oil & Propane Inc. the right to check the above references. The applicant's signature attests financial responsibility and willingness to pay their invoices in accordance to the terms listed above.
